Jock Itch

Jock itch, informally known as "itchy balls," is most commonly caused by a fungal infection that thrives in the warm, sweaty folds of the groin and inner thighs. The medical term for this is tinea cruris.

Despite its male-coded nickname, people of all anatomies can develop this condition.

In those with a vulva, it may also appear as a groin or inner thigh rash with similar symptoms.

Other Causes of an Itchy Groin

Many conditions can mimic jock itch, so it's worth knowing the difference.

Skin Conditions

  • Eczema (atopic dermatitis) - chronic inflammation that can flare up in skin folds
  • Psoriasis - appears in the groin as smooth, red patches (called inverse psoriasis)
  • Contact dermatitis - a reaction to something touching the skin, such as laundry detergent, fabric softener, soap, or latex

Infections (Non-Fungal)

  • Bacterial infections - erythrasma looks similar to jock itch but is caused by bacteria
  • STIs - herpes, pubic lice, or scabies can all cause intense groin itching
  • Folliculitis - infected hair follicles, often from shaving or friction

Parasites

  • Pubic lice ("crabs") - tiny insects that live in coarse body hair
  • Scabies - microscopic mites that burrow into skin and cause intense itching, especially at night

Lifestyle & Mechanical Causes

  • Chafing - friction from thighs rubbing or tight clothing
  • Heat rash - sweat trapped under skin in hot weather
  • Ingrown hairs - especially after shaving or waxing
  • Excessive sweating

Allergic Reactions

  • Condoms, lubricants, or spermicides
  • New underwear fabric or dye
  • Scented hygiene products or wipes

Less Common but Worth Knowing

  • Hidradenitis suppurativa - a chronic condition causing painful lumps in skin folds
  • Lichen sclerosus - more common in people with vulvas; causes patchy, itchy skin
  • Intertrigo - skin irritation and breakdown in moist skin folds

Symptoms

  • A spreading rash starting in the groin crease, moving down the upper thigh and buttocks
  • A rash whose center clears as it spreads outward
  • A fully or partially ring-shaped rash
  • A rash bordered with small blisters
  • Itchiness and scaly skin
  • Rash color may vary - red, brown, purple, or gray depending on skin tone

Preventing Jock Itch

1. Stay Dry

  • Dry off thoroughly after showering, especially in skin folds
  • Change out of sweaty or wet clothes as soon as possible
  • Use antifungal or talcum powder if you sweat heavily

2. Wear the Right Clothes

  • Loose, breathable cotton underwear
  • Moisture-wicking gear for workouts
  • Fresh underwear daily or more often if needed

3. Basic Hygiene

  • Shower after exercise
  • Use a mild, fragrance-free, pH-balanced cleanser
  • Never share towels or clothing since the fungus is contagious

4. Watch Your Athlete's Foot

Athlete's foot and jock itch are caused by the same fungus. Always put socks on before underwear to avoid transferring the fungus upward and treat athlete's foot promptly.

The simple rule: Jock itch loves heat, moisture, and darkness, keep things cool, dry, and breathing and you're mostly in the clear.

When to See a Healthcare Provider

Over-the-counter antifungal cream handles most cases, but see a healthcare provider if you experience:

  • Pain (not just itching)
  • Fever
  • Broken or bleeding skin - this raises the risk of secondary bacterial infection
  • Itching that is uncontrollable or getting worse
  • No improvement after one week of over-the-counter treatment
  • Rash that hasn't fully cleared after three weeks of treatment
